Open Hearts Program
Do You Know A Young Person Who:
BEHAVIORAL HEALTH Services:
Individual counseling, crisis intervention, skill building, family therapy, behavior modification, substance abuse counseling, psycho evaluation services, community integration, expanded referrals services, and free transportation to services if qualified. Services are provided by a diverse staff.
The program includes a special emphasis on:
Connecting male and female youth with Certified adult mentors;
Tours of juvenile detention centers, jails, and police stations;
Rehabilitative supports to prevent the need for an out of home placement and/or more restrictive services.
Services are conducted in community facilities, homes, schools and in YPM's offices.
All services are confidential.
PROGRAM QUALIFICATIONS
Youth must be 5 to 18 years old and enrolled in school.
Youth are also accepted who are under the supervision of DJJ (Department of Juvenile Justice); DFACS; or who are adopted, in foster care, group homes or receive Social Security Disability.
Payment options:
Medicaid, Wellcare, PeachCare, Amerigroup, private insurance, sliding scale and no income families.

- Needs more caring Black males in their lives?
